This program is NOT covered by insurance. But, if other diagnoses are found besides obesity then your insurance will be filed and you will be responsible for the deductible if it has not been met. Labs are usually covered on your insurance. But, we are not responsible for checking this out for you. Please make sure you understand your insurance plan before coming to the office. Once again, the fee above will be charged + any copays/deductibles you are responsible for. Payment plans are available.
